Here at Good Day Total Health Clinic, we  strive to give you our very best in order to enhance your spa experience with us. We adhere to certain policies to benefit you, and create a harmonious environment where our staff can focus on your needs. We know you will have a pleasant experience with us as we strive to exceed your expectations! Due to the popularity of our unique services, we highly recommend you make an appointment in advance for any of our relaxing spa treatments. However, we will always do our best to accommodate your requests. A major credit card is required to hold your appointment(s).

48-Hour Cancellation Policy
If you need to cancel a appointment or reschedule one, we require 48 hours notice for individual services and packages. Appointments cancelled or rescheduled in less than the required time will be billed 100%. Late arrivals will result in a shortened appointment.
Appointments booked for the same day are considered confirmed and cannot be cancelled, to adhere to our strict 48-hour cancellation policy.

Appointment
Please be advised that we require a credit card number or gift certificate number to secure your appointment at the time of booking.  We will require a valid credit card number for all first time appointments. You may choose to either pay with cash or credit card at the time when services are rendered.

First Visit
Arrive 15 minutes prior to your first treatment to allow ample time to check-in and prepare for your treatment. Arriving late will simply limit the time for your treatment, reducing its effectiveness and your pleasure. Your treatment will end on time so that the next guest is not delayed.
Ensuring our guests' safety is crucial. A 'guest-check' form must be completed after each and every treatment ends.

Guest Policies
All spa guests must be 14 years of age or older. Guests under 18 years of age must be accompanied by a parent or guardian.

Please advise us upon booking of any allergies, ailments, disabilities, high blood pressure, pregnancy or if a wheel chair access is required. Consumption of alcohol before, during or directly after particular spa treatments such as massages, body treatments and facials is not recommended.

AGE REQUIREMENT
Spa guests must be 18 years of age or older to receive treatments without the approval of a parent/ legal guardian  and/ or  accompaniment of an adult.

QUIET ENVIRONMENT
As a courtesy to other guests, please maintain a quiet level of conversation. Cell phones and pagers must be turned off when entering the spa. Failure to comply can result in client termination with no refund.

VISITORS
We are unable to accommodate visitors accompanying clients in treatment areas of the spa; including those complying with our age restriction policy. All non-clients must remain in the waiting lounge.

SPECIAL CONDITIONS
Please notify our Spa Consultant before reserving your treatments if you have pregnant, have muscle or joint injuries, are suffering from high blood pressure, special physical concerns, diabetes are using Retin-A, Avage, Tazorac, Differin, Accutane, any photosensitizing or keratolytic medications, or have had recent laser or plastic surgery. Any of the above conditions may affect our ability to perform your requested treatment. All spa guests will be asked to complete a brief questionnaire upon check-in, designed with your well being in mind.

PAYMENTS & GRATUITY
Good Day  accepts cash, MasterCard, Visa, American Express and Discover as payment. No personal checks are accepted. All gratuities may be paid directly to the aesthetician/clinician in cash, or can added to your credit card. We proudly recruit and train our staff to be the most qualified spa professionals in the industry. Our recommended standard gratuity for spa treatments is 15%-20%.

Personal Possessions:
Please keep any valuables with you for the duration of your visit. Unfortunately, we cannot be responsible for lost or stolen items.

Return Policy:
We will be happy to accept returns on any or most merchandise purchased from the spas within 10 days of purchase with a valid receipt: Unfortunately and without exception, we cannot accept opened products. When items are purchased as a set, we can only accept returns of complete sets, not individual items. All refunds are for SPA RETAIL CREDIT ONLY to be used on  future product purchases. We do not offer cash refunds. The spa does not accept returns on items purchased from other spas or establishments.

GIFT CERTIFICATES/Cards (SEE ALSO TERMS & CONDITIONS)*
Gift Certificates/Cards are available for specified services, packages, or dollar amounts beginning at $50 and are not redeemable if lost or stolen. If you are a recipient of a donated or auctioned Gift Certificate/Card, please give our receptionist the certificate/Card number to book your appointment. Donation certificates are non-transferable, and expiration dates may not be changed or extended. If appointment made to redeem your spa gift certificate is not cancelled at least 48 hours in advance, your spa treatments and your spa gift certificate will be forfeited. Gift cards cannot be returned or refunded. All sales are final for gift cards. All gift cards purchased after May 1, 2010 expires 1 year from purchase date. Gift cards purchased prior to this date adhere to our 1-year expiration policy. Expired gift cards can be reactivated for a fee of 10% of the card’s balance. Upon reactivation, the expired card must be used in its full value on your next visit, any remaining balance will be voided. Gift cards are treated as cash, all lost or stolen gift cards are not replaceable.

PRICES SUBJECT TO CHANGE
We are constantly expanding our services to bring you the latest and greatest. Although we make every effort to keep our website and spa menu updated, please note that prices and services are subject to change at any time. We love having you as a client, but we do reserve the right to refuse service at any time, to anyone, for any reason.

SERVICE PLAN POLICY:
A variety of service plans are available in Yoga, Pilates, Acupuncture, Esthetic, Therapeutic Massage, or Endermologie. Services/programs are not interchangeable, can not be distributed to anyone else. Programs will expire six (6) months after the start date.

CLIENTS RIGHT TO KNOW
We strive to exceed our client’s needs. We believe that advising our clients of the products best suited for them is as essential as the service we perform. Therefore, our client's right to know policy advises each client on the products and techniques best suited for them to maximize the benefits of their service.

THERAPISTS
Would a male therapist make you blush? Do you insist on a female with a lighter touch? If you have a preference for the gender of your therapist, please state it when you book your appointment. While we will make every effort to meet your requests, we cannot guarantee a specific therapist or gender especially for same day appointments.. All therapists are trained in both treatment and draping protocols to ensure your comfort at all times.

SPECIAL EVENTS:
We are happy to accommodate large parties. Please contact one of our receptionists to plan a special birthday party, baby shower, bridal event, etc.

We believe that a clear definition of our office policy will allow you, the patient, and us, Good Day Total Health Clinic, to concentrate on the bigger issue which is regaining and maintaining your health.

Good Day Total Health Clinic believes in the value of wellness care for you and your family and have maintained a pricing structure that allows care for all budgets. Our office participates in most insurance programs. We also accept auto accident and workers compensation cases. For patients who have little or no Chiropractic/ Acupuncture insurance coverage, flexible payment programs can be arranged. All fees are dependent upon services rendered and are ultimately the responsibility of the patient. Treatment plan cancelling or rescheduling:
When cancelling or rescheduling a treatment, please keep in mind that it is the frequency of your visits and not the days that will help in treating patients. It is the patient’s responsibility to make up any missed appointments within 7 days of any cancellation. We attempt to honor all appointments at the scheduled time however if you are late, you may have to wait until the next available appointment due to our other appointments. When entering the office, please go directly to the front and to “check in”. If there are any questions, please ask the front desk assistant.

24-Hour Cancellation Policy
If you need to cancel a reservation or reschedule one, we require 24 hours notice. Appointments cancelled or rescheduled in less than the required time will be billed 100%.

FINANCIAL POLICY:
Insurance
Most insurance plans are accepted at our office. We accept auto accident, workers compensation, personal injury and most health insurance plans, including Medicare. Since there are so many plans and coverage varies widely, please call us to find out if your plan covers care in our office. Our staff will verify and explain your benefits before treatment, at no charge once you become our patients. Deductibles and all co-payments are expected at time of service.  The following are accepted insurance plans, but may not be limited to:


Non Insurance
For patients who have little or no Chiropractic/ Acupuncture insurance coverage, flexible payment programs can be arranged. If you have no insurance or do not have Chiropractic/ Acupuncture benefits there is still a way for you to receive the care you need. All payments are expected at time of service. If you have financial hardship to cover medical expenses please contact our billing department for financial aide program.

Family Plan
Our focus at Good Day Total Health Clinic is to actively help you and your family in taking responsibility for your health, wellness, and disease management. We assist and guide you to take the time to care for the important people in your world: you and your family.  In an effort to make living the wellness lifestyle as easy and affordable as possible, we can create a customized family plan that is specific to you and your family.

Other:
Any balance past due 90 days will be sent to our collection lawyer and an additional collection fee is added onto your balance. Also there is a $40.00 service fee for any returned checks.
Patients will be  considered to be a non insurance patient until our office qualifies your coverage to determine the extent of benefits under your policy.
All patients whose visitation schedule is once per month (or longer) will not be eligible for insurance assignment unless otherwise stated. Charges for services rendered will once again be due as they are received.
Should you discontinue care for any reason other than discharge by the doctor, any and all balances due will become immediately payable in full, regardless of any claims submitted.
This office does not promise that an insurance company will reimburse you for the usual and customary charges submitted by this office, nor will we enter into any dispute with an insurance company over the amount of reimbursement.
Since we do not own your policy, and, occasionally, we experience difficulty in collecting from the carrier, we may ask for your active assistance in rectifying this situation.
